Address: [redacted] Gateshead Drive N. Chesterfield, VA [redacted] May 26, [redacted] North Star Surgery Urgent Specialty Associates [redacted] Jahnke Road Richmond, VA [redacted] Dear Office Manager, I underwent an emergency appendectomy at Chippenham Hospital, Richmond VA on March 9, [redacted], where Dr. Shannon Small and the team from your practice provided care. I recently received a "collection letter" from Capio Partners, Dallas TX, dated May 21, regarding my account. Surprisingly, I had not received any bills until mid-May. After multiple inquiries, I managed to identify and pay the $[redacted].31 bill to your company with check #[redacted] on May 16, [redacted], which cleared on May 20, [redacted]. I am displeased with the fact that the bill was sent to a collection agency without prior notice despite prompt payment. Please ensure that any negative impact on my credit is rectified promptly. I also received another bill for additional services from Dr. Small, but have been unsuccessful in reaching Urgent Specialty Associates for clarification, which is concerning given the lack of transparency in billing practices today. I request a swift response to address these concerns. Sincerely, Jane R. Logan

Hello, I'm Brittany Rogers. I'm reaching out because I've been attempting to contact Equifax for customer support, but the wait times are extensive. I have an account freeze that needs to be lifted. As I am preparing to move into my inaugural apartment and set up utilities under my name, I discovered that my credit account is frozen. I have a time crunch of 5 days to address this matter. It's crucial for me as this is my first apartment, and I'm eager to complete the move-in process promptly. Your assistance in resolving this issue is greatly appreciated. Thank you.

I recently discovered that Social Security uses Equifax for identity verification. I am encountering issues online with Social Security, possibly because my credit card displays my first and middle name in reverse order. My birth certificate and Social Security card show my name as Roy Virgil Hibbs III, but I commonly use Virgil on my credit card and in various other places. Is there a way to link these two names? My address remains consistent, whether registered as Virgil or Roy, and my mother can confirm our shared residency. I am eager to resolve this for Social Security and Medicare purposes. Your assistance in resolving this matter would be greatly appreciated.

I recently received a letter from Equifax requesting additional information for me to access my credit report. They are asking for utility bills, mortgage confirmation, and a W2 form for current address verification. Unfortunately, I do not have any of these documents. Would it be acceptable to send a copy of an envelope with my name and present address as proof instead? I previously provided Equifax with my Social Security Number and state ID to verify my identity. I am also having trouble accessing my Social Security account as the information provided does not match what they have on file. I am concerned about potential fraudulent activity using my SSN, and I am struggling to get assistance.

I recently received a call from someone claiming to be from AT&T, stating I needed to upgrade my DirecTV box. Unfortunately, it turned out to be a scam. The individual tried to remotely upgrade the box on July 23 and 27, but AT&T confirmed it didn't happen. They also mentioned sending someone to replace the box that was installed in November, which raised suspicions. After canceling, the caller persistently warned me of losing TV connection unless I allowed the upgrade. Regrettably, I fell for the scam due to being 86 years old. The scammer requested a $99.00 check to be sent to Bill at PO Box [redacted], Springfield VA [redacted], with "SAM" written on the lower left corner. I've taken steps to safeguard my accounts but remain concerned about potential harm. My neighborhood's poor cell phone reception further complicates the situation.

I recently received a letter claiming to be from Equifax, but I noticed some discrepancies. The address provided is outdated, and the overall presentation seems unprofessional. My legal name is Rebecca, even though I go by Becky informally. I am concerned about the request for an ID with an old address. This seems suspicious, especially since my ID was recently stolen. I worry that someone might be attempting to use this information fraudulently. I wanted to reach out to address these concerns. My contact number is [redacted], and my current address is [redacted] Van Patten, Las Vegas, NV [redacted], Apartment 11B.

I am unable to access my account since August 5th. Each time I try to log in, I am prompted to call their [redacted] number. Despite them recognizing my account, they do not accept my password. I am unable to create a new account and continue to receive unhelpful support. I have contacted the help desk three times without a resolution: - On 8/5, the help desk claimed it was a system issue and mentioned they would email when it's resolved. - On 8/13, a representative stated the system was down for upgrades and asked me to call back later. - On 8/20, I faced disconnection during the call and was given repetitive excuses about system upgrades and password resetting issues. I am requesting assistance to either fix the password reset feature on the website or have a representative manually reset my password.

On my credit report dated August 26, [redacted], it was indicated that the key factor affecting my credit score negatively was having too many accounts with balances. I have only two accounts with a total balance below $[redacted], as I make sure to pay off all my revolving accounts entirely each month. I would appreciate an explanation as to why having two accounts with such a low balance has had a detrimental impact on my credit score. I am struggling to comprehend how this situation is not considered absurd.

In March [redacted], my husband's work hours were reduced due to COVID-19. I reached out to both Wells Fargo and GMC to request a deferment on our vehicle payments. Wells Fargo allowed a 2-month deferment, while GMC approved a 3-month deferment. I assumed everything was in order. However, in June, just before my next payment was due (the first after the deferment period), I received a letter from Wells Fargo stating that I was 3 months behind on payments. I called them to address the issue, and they acknowledged a mix-up on their end and promised to correct it. We also noticed a drop in our credit score, which I inquired about. They assured me they would contact the credit reporting agencies to rectify it within 30 days. It has been nearly 60 days now, and the credit score issue remains unresolved. I am uncertain if further action is necessary.

I attempted to place a temporary freeze on my account, but instead, I got locked out. After multiple attempts and several calls to customer service, I still cannot access my "MyEquifax" account. A representative on August 19th mentioned a ticket number [redacted], but I have not received any updates. During my recent call, after a 25-minute verification process, the call abruptly ended, and I was prompted to take a survey. I have also tried to reset my password online, but I am not receiving the necessary email. I first need to ensure that my email address is correct (provided below) for responses. Then, I urgently require the email containing the password reset link to be able to access my account and update my address due to a recent move requiring a credit check. Kindly let me know if you need any additional information. - Lewis I.

I have been attempting to temporarily remove a security freeze, but after several failed online attempts resulting in the message to return to the dashboard and try again, I am frustrated. Phone calls have also gone unanswered. I expected a smoother process online since phone support seems limited. Experian and Transunion were able to lift my freeze quickly without issues, unlike my experience with your company. I am unable to proceed with any actions on my account until this freeze is lifted, and the current situation is quite inconvenient.

On September 9, I sent a letter to Equifax with my driver's license and SS# to request a credit freeze lift. When I called on September 28, the freeze was temporarily lifted until October 2 after answering some questions. However, when I contacted Dish, they couldn't proceed due to a message saying my credit file didn't match the information submitted. Despite paying about $3,[redacted] monthly on my credit cards, it seems this may not be enough credit buildup as I have no other debts and own my home and car outright. I wonder if my credit card payments aren't impacting my credit score positively. Removing the statement on my file would have possibly approved my Dish installation. I've requested a copy of my Equifax report to review. Thank you for your assistance.

A salesperson from Sprint informed me that the phones I was acquiring were prepaid, so I decided to lease two iPhones - an iPhone 7 and a Motorola, which were supposedly free. They also promised me a $[redacted].00 promotional gift card, which I never received. I also leased an iPhone 10 or XR and an Apple Watch series 4, for which I paid in full. I maintained a good credit report by paying my bills on time every month for over two years with Sprint. Towards the end of my contract, my monthly bill increased to $[redacted].00, and although I was informed I was under a contract, I decided to stay for two more weeks as advised by the manager. It came as a shock when I was charged over $1,[redacted], with no clear explanation from Sprint. I have always been dedicated to paying my bills promptly to improve my credit score, yet I am still struggling to get loan approvals. I even enlisted the help of Financial Education Services for two years, but unfortunately, I have not seen any progress from their services.

I am having trouble with my Equifax account for Social Security verification. After being instructed by agent Richter to send a letter and ID pictures to PO Box [redacted], I have not received any response or resolution to my queries. While I did receive confirmation of my Equifax account setup on 10/2 with verification code 9TZ3C9JZZS, I am now unable to log back in. I need to resend my information and require an email address to do so in order to activate my Medicare after a surgery at StonyBrook Medicine NY. Thank you for your help.

I recently received a bill from the SBA (Small Business Administration) stating that I owe $[redacted] for a federal assistance loan, even though I don't own a small business. When I contacted SBA, I was informed that due to Covid, loan companies were being more lenient and only required basic personal information to apply. Since I'm not a business owner and I don't have $[redacted], I am taking steps to protect my credit by freezing it and reporting the incident to the OOIG (Office of Inspector General) and SBA.gov. I have also contacted Social Security to inform them of the security breach involving my social security number. I am eager to receive assistance from your office to review my credit report for any suspicious activity. Your prompt attention to this matter would be greatly appreciated. -Tara Cabney

I recently received a bill from the SBA (Small Business Administration) stating I owe $7,[redacted] for a federal disaster loan. Despite not owning a small business, I learned that someone used my information to apply for the loan during the pandemic. This incident has prompted me to take measures to secure my credit, including freezing it and contacting the OOIG (Office of Inspector General) and SBA.gov. I plan to inform the Social Security Administration about the misuse of my social security number. I am concerned about other fraudulent activities and would appreciate a thorough review of my credit report. Your prompt attention to this matter would be greatly appreciated.

I have had a long history of being mistreated by my water supplier, Thames Water. They have consistently overcharged me despite their own survey confirming that I am not using the services they claim I am. This ongoing dispute has caused me distress, especially due to my complex health issues. Additionally, the aggressive tactics used by both Thames Water and their debt collection agencies have only added to my frustration. It is important to note that the evidence from their own survey does not support their claims against me as a customer.

I attempted to unlock my account around 12:30 on 11/17/20. After struggling for hours to reach a live person, I finally connected with someone. Although the representative's English was hard to understand, they assured me that my account would be unlocked immediately. However, by 3:00, it was still locked. The confirmation number I received is [redacted]. I am in urgent need of help! Additionally, I am looking to update my username and password for future access to my account, as the current ones are no longer working. My name is Weldon Curtis Patterson. You can reach me at [redacted] or [redacted].

My username and password are incorrect, and my account is now locked after multiple failed attempts. I tried to recover my account, but I'm currently unable to do so. I need to access my account using either a temporary password or another method. Another major concern is that my account is locked, but I need to undergo a credit check in mid-December. Can my account please be unlocked as soon as possible? I have sent an issue statement and supporting documents (such as my SS card and driver's license copy) via priority mail to the GCS Atlanta support team yesterday morning, November 23rd.

My wife is experiencing memory problems, and I need to close our joint Citicard Visa account, where she is the primary account holder, due to her inability to manage financial matters. I require a new card in my name, possibly with her as a secondary user for occasional shopping trips while I drive since she can no longer drive. I visited Costco to address this, but they advised me to unfreeze my credit to proceed. Being 84 years old, I struggle with following computer instructions. Can you provide assistance with this process?
