I am a VERY FRUSTRATED PROVIDER....I need to have the address where I receive remittances changed. I have submitted in writing to PROVIDER DATA SERVICES *x. The first time I received back correspondence telling me that i needed to submit a W* Form. The second time I submitted a W* form and received the same correspondence back that I needed to submit the form (which I did). My remittances are going to an address at which I no longer have access. I need desperately THE CORRECT INFORMATION to get my address changed and not be stalled again. My ta x id is *********.
